This form is to be used from 1 December 2026. This is provided earlier to help village operators prepare for the new laws.
Operators must use this form to prepare a modification plan for proposed changes to a retirement village that require residents’ consent.
The plan gives residents key information about the proposed modification before they consider and vote on it.
The modification plan must be provided to residents at least one month before the meeting. Any supporting documents, such as technical drawings, plans or extra information, must also be provided with the plan.
