- Fill out sections 1-9 of the form.
- If there is not enough room to provide an answer to a question, additional information and any relevant supporting documents can be attached to this form.
- Once completed, this form should be provided to all resident(s) still living in the village.
Don’t send this form to the Department of Local Government, Industry Regulation and Safety.
